Consent and forms
Medical history and consent, signed on the patient's phone before they arrive or on your iPad in clinic. Risky answers are flagged for a clinician before treatment.
The right forms, for the right treatment, every time
Each treatment has its own consent form. The full medical history is asked every six months, with a quick "has anything changed?" at the visits in between.
- Answers that need a look are marked to review on the patient's record
- Forms not done show on Today, so nobody is treated without them
- Patients sign with their name, date and time recorded

A change makes a new version. Each patient keeps the version they signed.
